OK. Just checking, because some people have games installed on a separate physical disk that they simply link to the location after a clean install of Windows.
At this point, I'd triple check all of the drivers for all of your hardware, making sure they are as current as possible, and not just running the standard built in drivers from Windows. I'd also try manually uninstalling the game and reinstalling it again (or, if your game platform allows for it, run a file 'verification' utility, at the very least). My suspicion is now mostly on either driver incompatibility, or else perhaps some sort of anti-cheat software that is simply not working correctly.
