Try doing the updates, latest Build I have is 22631.3737.

Open a command prompt as Admin, and run these commands, one at a time.
Wait for the first one to finish before running the 2nd one.

sfc /scannow

Dism /Online /Cleanup-Image /RestoreHealth

I don't know, sometimes the Explorer's appearance is fine, and sometimes it changes like that. Maybe I opened Explorer too quickly, I'm not sure. I'm not using any mods, just debloating the default Windows applications.

Try clearing File Explorer cache, see that will help.

Reinstall UI3 elements and reboot. See if that fixes explorer:

Thank you to everyone who made suggestions. It looks like my explorer is now working fine using the methods from @Berton, @FreeBooter, and @dacrone. Sometimes the bug reappears, but I can restart the explorer using the right-click context menu method from @Edwin. Thanks again, everyone! Thread closed.
