I had weird issues with 4K monitors and mixed resolutions, as well. I finally got rid of all the 4K's and just went with all 1080 Ultra-wide's! 🤷‍♂️

It might be that the last updates also replaced your graphics drivers leading to bugs. Make sure you download and install the latest drivers directly from manufacturer (Intel, nVidia, AMD). If these official drivers don't work (rare they usually work) due to customized hardware, download the latest graphics driver directly from your computer or motherboard manufacturer.

I would try to change the scaling to 100%, wait a few seconds, and then change it back to 125% or whatever it was before. This should force all icons to resize accordingly. Yes, and the original point being that everything was working/displaying absolutely perfectly fine before and some update or the other messed it up. The issue is not you or your setup.

It just so happened that Intel released new drivers today itself! :)

I installed them, but still no change for my desktop. :(

Hello, I just wanted to update everyone: after the reboot with the updates in the screenshot above, my desktop icons are back to normal. YAY!

Many desktop apps still have microscopic icons though 🧐
